  50. Please read all of the information below.  It will explain all of your ordering options.  All of the information you supply on this form is confidential.

  53. This is the name under which you would like to game to be registered.

  55. Address
  56. This is your mailing address.  This is where we will send the Unlock Code if you do not give us an email address.

  58. Phone
  59. Optional.  This is your phone number.  We will only call you if there is a problem with your registration.
  60.  
  61. EMail
  62. This is you email address.  This is where we will send the Unlock Code.  
  63.  
  64. Quantity
  65. This is the number of copies of the game you would like to purchase.  Each copy will have it's own Unlock Code.

  67. Total
  68. This is the total amount of money you are sending to us.  This number is calculated as the Quantity multiplied by US$10.

  70. Cash Orders
  71. We do not recommend that you send us cash.  If you must, be sure to double-envelope the cash to make it less obvious.  All cash must be in US currency.  We cannot be held responsible for cash lost in the mail.

  74. Checks must be in US funds drawn on a US bank.  Please make all checks payable to "Stick Man Games, Inc.".  There will be additional processing time on all check orders as we must wait until the check clears.  There will be an additional US$10 charge on all bounced checks.

  77. Money orders must be in US funds.  Please make all money orders payable to "Stick Man Games, Inc.".

  80. After your registration has been processed, we will send you an unlock code.  If you gave us your email address, we will email you the code, otherwise we will send it via US mail to the address you give us.
